The Clinical Care Partner role, available on a part-time or full-time basis with Action on Disability within Ethnic Communities Inc (ADEC) in South Morang, VIC, offers a rewarding opportunity to make a meaningful difference in the lives of clients within the aged care sector. This role is ideal for a compassionate and dedicated professional committed to delivering high-quality, person-centred care to diverse communities.

This position will also lead our service programs through a period of transformation, aligning service delivery with the new Support at Home Program (SAHP) Framework and funding model. As part of a dedicated team, you will drive excellence in service quality and client experience while fostering program growth.

Key Selection Criteria

The incumbent must: 

  • Be a registered nurse with demonstrated experience in a similar role as a Clinical Care Partner/Case Manager.
  • Have extensive understanding of the challenges, needs, and issues affecting older people, their families, and carers.
  • Have demonstrated knowledge and understanding of the Support at Home Program, and support systems within the aged care sector, including funding mechanisms and associated requirements.
  • Have proven experience in delivering person-centred care and support with the ability to work within service models that promote client empowerment.
  • Capacity to work with a diverse range of people, including people with a disability, diverse communities, allied health and other service providers, assessment agencies and referrers.
  • Excellent interpersonal skills and ability to work in a multidisciplinary team.
  • Ability to genuinely embody and promote our values.
  • Must be available on Mondays, Wednesdays and Fridays.

About the Role

You’ll play a vital role in enhancing your clients’ independence, wellbeing, and connection to their communities. Every day will bring new opportunities to make a positive impact.

Your responsibilities will include:

  • Assisting with personal care: showering, dressing, grooming.
  • Supporting domestic tasks: cleaning, laundry, cooking, and light home maintenance.
  • Providing transport to appointments, social outings, and community activities.
  • Encouraging and participating in social, recreational, and cultural engagements.
  • Keeping accurate records and communicating effectively with support teams.
  • Advocating for clients’ rights, needs, and preferences.

What We’re Looking For

Ideal candidates will have:

  • A Certificate III in Individual Support (Aged Care or Disability), or similar.
  • At least 6 months of hands-on experience in home care, Aged Care or disability support.
  • A caring, respectful, and flexible approach to client care.
  • Current First Aid certification, a valid driver’s licence, and access to a reliable vehicle.
  • Provisional driver’s licence or higher required.
  • Australian work rights required at the time of application.
  • Both Working with Children Check and Police Check required.
