Applying for CHSP services involves a straightforward process that begins with a face to face assessment of your eligibility and needs. Here’s how to apply for CHSP:

1. Check Your Eligibility: Ensure that you meet the eligibility criteria based on your age and level of need.
2. Apply through My Aged Care: Access the My Aged Care website or call the My Aged Care contact centre to initiate the application process.
3. Complete the Assessment: Undergo an aged care assessment conducted by either the Regional Assessment Service (RAS) or the Aged Care Assessment Team (ACAT) to determine your eligibility and specific needs. MAC will be in contact to start this process.
4. Develop a Support Plan: If approved for CHSP services, the assessor will work with you to develop a personalised home support plan tailored to your needs and goals.
5. Choose a Service Provider: Use the My Aged Care Service Finder to find available providers in your area. ADEC is your trusted HCP and CHSP service provider.
6. Organise Services: Once you have chosen your service provider, they will work with you to arrange the services you require, coordinate with ADEC to arrange the services you require.
7. Review and Adjust:  Your service provider will regularly review your support plan and services to ensure they continue to meet your needs and goals.

By following these steps, you can initiate the process of accessing CHSP services and begin receiving the support you need to maintain your independence and well-being at home.

• You are an Australian aged 65 or over
• 50 or over for Aboriginal or Torres Strait Islander peoples
• Have functional limitations and need assistance
• You must be registered with My Aged Care to avail these services.

• Transport– help to get out and about into the community such as attending medical appointments, shopping or visiting family and friends.
• Domestic assistance– General house cleaning such as vacuuming, mopping, and cleaning the bathroom.
• Personal care– help with bathing or showering, dressing, hair care, and going to the toilet.
• Specialised support services – Assistance with registration to My Aged Care and guidance to access services through other CHSP providers. Please read more about it here.

CHSP Direct Care Service Client Co-Contribution (ex GST)

1. Domestic Assistance – $10.00 per hour
2. Personal Care – $10.00 per hour
3. Transport (Local) – One-way single trip. ADEC covers the first $20.00 of each trip thereafter it is client contribution.

The above fees are current as of 01 July 2024. Fees are reviewed annually and are subject to change. All services are arranged and approved in consultation with you. The services you receive are based on your assessed needs and the established goals in your Care Plan.

• We provide Personal Care, Domestic Assistance, and Specialised Support Services in the Northern Region.
• We also offer subsidised Transport services in the Western, Eastern, and South-eastern regions.

ADEC cannot offer CHSP services outside the above mentioned areas.

While CHSP funding is intended to support individuals in maintaining their independence and safety at home, there are certain restrictions and limitations on how the funding can be used. CHSP funding cannot be used for services that fall outside the scope of the program or for services that are already covered under other government-funded programs. Additionally, CHSP funding cannot be used to cover the cost of permanent residential aged care or services provided by unauthorised service providers.
