Once we receive the referral, our team will reach out to the individual or you to discuss their needs and create a personalised support plan.

Refer and be Rewarded
When you refer a friend or family member to our Aged Care services, you’ll receive a $100 Coles voucher as a thank-you once they sign up.
How It Works:
- Share the details of this offer with friends or family who could benefit from our Aged Care services.
- When they sign up, you’ll receive a $100 Coles voucher to enjoy.

Terms and Conditions for the Aged Care Referral Campaign

Eligibility
- This referral campaign is open to individuals aged 18 years or older.
- The referrer must not be an existing client of our Aged Care services at the time of the referral.

How It Works:
- To refer someone, call our hotline at (03) 9480 7000 or complete the following online referral form.
- Ensure all referral details (e.g., name, contact information) are accurate and complete.
- The referred person must sign up for and commence using our Aged Care services for the referrer to qualify for the reward.

Reward Details:
- The referrer will receive a $100 Coles voucher once the referred person has successfully signed up and started receiving services.
- Vouchers will be sent via email or post within 14 business days after confirmation of the referred person’s signup.

Campaign Duration:
- The referral campaign runs from 1st January 2025 to 25th June 2025.
- Referrals made outside this period will not be eligible for the reward.

Limitations:
- Only one referral reward ($100 Coles voucher) will be issued per referred person.
- There is no limit to the number of referrals a single participant can make, provided each referral meets the eligibility criteria.

General Conditions:
- ADEC reserves the right to verify the validity of all referrals and may request additional information from the referrer or referred person to confirm eligibility.
- Incomplete, fraudulent, or invalid referrals will not qualify for the reward.
- The $100 Coles voucher is non-transferable, non-exchangeable, and cannot be redeemed for cash.

- All personal information collected during this campaign will be handled in accordance with ADEC’s Privacy Policy.
- ADEC reserves the right to modify, suspend, or terminate this campaign at any time without prior notice.

Home Care Packages offer flexibility to include a variety of care, and services tailored to your specific needs. Collaborating with your provider, it’s essential to determine if a particular care or service aligns with the following criteria:
Direct Link to Care Needs and Goals:
Ensure that the care or service is directly associated with your identified care needs and goals, fostering a personalised approach to your well-being.
Health and Wellbeing Improvement:
Assess whether the care or service contributes to the improvement of your health and overall wellbeing, enhancing your quality of life.
Safety and Independence:
Verify if the care or service is essential for you to continue living safely and independently in the comfort of your home, promoting a sense of security.
Budget Alignment
Confirm that the proposed care or service can be effectively delivered within the constraints of your Home Care Package budget, ensuring financial feasibility.
Government Fund Appropriateness
Evaluate if the care or service would be considered an acceptable use of government funds, aligning with regulatory guidelines.
It’s crucial to note that while Home Care Packages are comprehensive, they should not replace care and services accessible through other health programs for which you might be eligible. Continuing to access these services, as required, through your GP and hospitals is essential to complement the holistic approach to your health and well-being.
Selecting the right service provider is crucial. The government subsidises your chosen provider to organise a tailored package of care services to suit your individual needs. Given the diversity in care requirements, there are four Home Care Package levels, each with varying funding amounts addressing basic to high care needs.
Package Levels:

Level 2: Low care needs
Level 3: Intermediate care needs
Level 4: High care needs
Your assigned package level depends on your unique needs. Collaborate with your chosen provider to assess your care requirements and strategically allocate your package funding.
