How do I apply for CHSP?
Applying for CHSP services involves a straightforward process that begins with a face to face assessment of your eligibility and needs. Here’s how to apply for CHSP:
- Check Your Eligibility: Ensure that you meet the eligibility criteria based on your age and level of need.
- Apply through My Aged Care: Access the My Aged Care website or call the My Aged Care contact centre to initiate the application process.
- Complete the Assessment: Undergo an aged care assessment conducted by either the Regional Assessment Service (RAS) or the Aged Care Assessment Team (ACAT) to determine your eligibility and specific needs. MAC will be in contact to start this process.
- Develop a Support Plan: If approved for CHSP services, the assessor will work with you to develop a personalised home support plan tailored to your needs and goals.
- Choose a Service Provider: Use the My Aged Care Service Finder to find available providers in your area. ADEC is your trusted HCP and CHSP service provider.
- Organise Services: Once you have chosen your service provider, they will work with you to arrange the services you require, coordinate with ADEC to arrange the services you require.
- Review and Adjust: Your service provider will regularly review your support plan and services to ensure they continue to meet your needs and goals.
