Home support made simple, care that feels like family
What is Support at Home?
Support at Home is a program to help older people remain at home as they age by delivering coordinated care and services to meet their assessed ageing-related care needs. An older person must first go through an aged care assessment to get an approval to receive Support at Home funding. They will then be allocated funding when it is available and enter into a service agreement with a provider they choose.

Who can get Support at Home?
You may be able to get Support at Home services if you are:
- Aged 65 years or older, or
- A First Nations person aged 50 years or older; and
- Have age-related care needs that make it difficult to live independently at home

What makes our Support at Home service different?
Our Support at Home service has been developed to ensure that what is important to you is well supported, and that you are able to live at home as independently, safely, and confidently as possible. We understand that quality care goes beyond services, it’s about trust, understanding, and personalised support. As we move into the new Support at Home model, our focus remains on delivering care that’s clinically sound, culturally sensitive, and consistent.
Multidisciplinary Team and Continuous Qualified Clinical Support
Every client is supported by their own dedicated Clinical Care Partner (CCP) — a qualified registered nurse who provides expert Care Management.
Our Clinical Care Partners work closely with you and your loved ones to develop a personalised service plan based on a comprehensive assessment of your needs. They coordinate with your doctor and any required allied health or therapeutic services to ensure you receive the best possible clinical support.
Your CCP will stay in regular contact, monitoring any changes in your health or circumstances to make sure your care plan evolves with you. This proactive approach helps you stay well and safe at home for longer, while giving your loved ones peace of mind and confidence in your ongoing care.
Culturally Sensitive Care
We know that cultural understanding is key to quality support. Our Service Coordinators are committed to matching clients with support workers who respect their cultural background and speak their language. We have a team fluent in over 33 different languages.
Continuity of Care
We believe strong relationships lead to better outcomes. That’s why we strive to ensure the same support worker provides assistance wherever possible, offering stability, trust, and comfort for every client.
Support workers employed by ADEC are permanent staff, not contractors. This means we can ensure they are qualified, regularly trained, background-checked, and well supported — so you can always count on receiving the highest standard of care.
Experienced Service Coordinators
Your dedicated care team also includes experienced Service Coordinators who ensure your services run smoothly every day. They’re your friendly, reliable point of contact on our Customer Support line — ready to help if you’d like to change a service, need advice, or simply want some guidance. With ADEC, you’ll always have a familiar, caring voice on the other end of the line whenever you need us.
Our Support at Home team can assist with
- Creating a personalised care plan tailored to your needs and goals
- Home safety assessments and advice to reduce risks at home
- Arranging home modifications such as ramps or grab rails to keep you safe
- Support when returning home after illness or a hospital stay
- Accessing aids and equipment to assist with daily living
- Referrals to allied health and other services you may need
- Advocacy for financial hardship or service access, ensuring you get the support you deserve
- Advocacy for increased funding if your care needs change
- Arranging volunteers for companionship and social connection
- Support and respite for family or friends who help look after you

Transitioning to Support at Home explained
On 1 November 2025, people receiving or assessed as eligible for HCP or STRC funding transitioned to Support at Home.
- Existing HCP clients – Everyone receiving a Home Care Package was automatically transitioned to the new Support at Home Program. Your care and services have continued seamlessly under this new system. All clients received information about these changes, but if you have any questions or would like further support, ADEC is here to help. Please contact our team anytime, we’re happy to guide you through what this means for you.

Support at Home services provided by ADEC
Clinical Support
Our qualified team provides expert care to help you manage your health and wellbeing at home. This includes:
- Nursing care, such as wound care and medication support
- Allied health services, including physiotherapy, psychology, occupational therapy, and dietitian support
- Care management, to help you get the most from your package and coordinate your services
- Emotional and psychological support, from professionals who understand your needs
Independence and Daily Living Support
We’re here to help you stay independent and confident in your daily life. Our friendly and professional team can assist with:
- Personal care, such as showering, grooming, and getting ready for the day
- Social support, to help you stay connected and engaged in your community
- Therapeutic services, including art therapy and massage, to support your wellbeing
- In-home respite care and companionship — day or overnight — giving your loved ones time to rest and recharge
- Transport support, to help you get to appointments, activities, or social outings safely
- Other therapies, to help you to live comfortably
Everyday Living Services
ADEC can help make daily life at home easier and more comfortable. Our team provides practical support so you can enjoy a safe and well-maintained home environment. This includes:
- Help around the home, such as cleaning, laundry, and general household tasks
- Minor home maintenance and repairs, to keep your home safe and in good condition
- Gardening services, to maintain your outdoor spaces
- Meal preparation and support, so you can enjoy healthy, fresh meals that suit your needs
Additional Short-term Care Programs
In addition to regular Support at Home services, ADEC can help you access short-term care programs designed to meet specific needs at different stages of life and recovery.
Restorative Care
This program provides up to 16 weeks of intensive support to help you regain strength, mobility, and daily living skills after a change in your health or hospital stay. Click here for more information.
Assistive Technology and Home Modifications (AT-HM) scheme
This program helps you access equipment and home changes that make everyday living easier and safer. This may include:
- Mobility aids such as walkers or shower chairs
- Home modifications such as grab rails, ramps, or step-free access
- Support is based on your assessed needs.
If you have been a Home Care Package client and have unspent package funds, you will be required to use these funds before the AT-HM program can be accessed.
