How to know if you’re eligible for Government-funded Aged Care Services

You could qualify for government-funded aged care services if you are aged 65 or older (or 50 and above if you identify as Aboriginal or Torres Strait Islander) and require assistance with daily tasks. In certain cases, younger individuals may also be eligible.

How to sign up for the Govt’s My Aged Care Services

To access any government-funded Aged Care Services, registration with My Aged Care is mandatory. If you need help with setting up your ‘My Aged Care’ program. Please call ADEC at (03) 9480 7000 and request to speak to our Aged Care Team, we will assist through the entire setup process.

 

Step 1: Register with My Aged Care

Call 1800 200 422 to register.

Step 2: Assessment by My Aged Care Team for a Home Care Package

My Aged Care Team member will contact you to discuss your home support needs. The assessment can be conducted over the phone or at your home, with the option for family or friends to attend alongside you for support.

Step 3: Receive the outcome of your assessment

You will receive a letter from My Aged Care assigning you a Home Care Package Level 1, 2, 3, or 4. Please note that there is a 6-12 month waiting period to receive your Package.

Step 4: Assignment of a Home Care Package and “Referral Code”

My Aged Care will send another letter confirming your assignment to a Home Care Package. On page 2 of this letter, you will find a referral code. Kindly provide this code to your Service Provider.

Step 5: Get started with ADEC

ADEC is an Aged Care Service Provider, call us at (03) 9480 7000 to get started.

Step 6: ADEC will assign you a Care Manager

Your Care Manager will oversee your package and set up your services in accordance with a personalised care plan, which may include services such as; cleaning, gardening, meals, personal care, house maintenance, nursing, physiotherapy, podiatry, occupational therapy, dietitian services, medication support, and taxi services.

Approved on or after 12 September 2024

If your Home Care Package was approved on or after 12 September 2024, you are already being assessed under the new Support at Home rules.

  • You’ll receive one of eight classification levels, with annual budgets ranging from $11,000 to $78,000, based on your assessed care needs.
  • You’ll start with a quarterly budget from the beginning.
  • You can carry over up to 10% of your quarterly budget or $1,000 (whichever is greater).
  • Any unspent funds in your package before 31 October 2025 will also transfer into your Support at Home account.
  • You’ll follow the new participant contribution model from day one.

Approved before 14 September 2024

You will be automatically moved into the Support at Home program from 1 November 2025, no new assessment is needed, as long as your care needs haven’t changed.

  • You’ll be placed into a Support at Home classification that closely matches your current Home Care Package level.
  • Your funding will shift from an annual to a quarterly budget.
  • Any unspent funds in your current package will carry over into your new Support at Home account.
  • From November 2025, you can carry forward up to 10% of your quarterly budget or $1,000 (whichever is greater).
  • You’ll move to the new participant contribution model, based on your income (full pensioner, part pensioner, or self-funded retiree).

The new Support at Home program introduces eight new funding classifications that give people different amounts of money, from $11,000 to $78,000 annually.

While there is no extra funding for people from multicultural or non-English speaking backgrounds, ADEC will continue to support your cultural and language needs as we always have. This is the reason we are here!

If you need some short-term help, like getting better after being in hospital, or you need palliative care, we can help you apply for extra funding through two new schemes under the Support at Home program called:

  • The Restorative Care pathway
  • The End of Life pathway

If you need any equipment or major home modifications, we can help you apply for extra funding under the Assistive Technology and Home Modifications (AT-HM) Scheme. However, any unspent funds will have to be used first.

Care Management is important to help you get the most out of your service plan and to help you stay living in your own home comfortably and safely.

Care Management activities include:

  • Assessing your needs, goals, and risks.
  • Planning the best supports and services to meet your needs with you (care planning).
  • Helping you get the best value out of your package funds.
  • Arranging and monitoring the services you need under your care plan (example Physio, gardening etc).
  • Helping you adapt to changes in your health or abilities.
  • Regularly reviewing your care plan with you and communicating with you and others as necessary about your care and support.

As an ADEC client, you will have a Clinical Care Partner, who is a qualified nurse, to assess your needs and clinical risks and help you plan to meet your goals.
