What is NDIS Transition Support?

The aim of ADEC's NDIS Transition Support Services is to assist people with disabilities, their carers, and their families, who are from culturally and linguistically diverse backgrounds, to transition to the NDIS.

Our focus is to build the capacity of NDIS participants and their family to confidently
navigate the NDIS. ADEC provides tailored workshop on topics related to the the NDIS, Cultural awareness, Human rights, Identifying abuse, and Self advocacy, for culturally diverse communities as well as service providers.

  • Information regarding eligibility for NDIS
  • Guidance and support through access requests
  • Assistance to gather quality supporting documentation from Medical and Health professionals
  • Education about clients' rights and responsibilities
  • Advocacy for access to necessary services, connecting clients with the right individuals for support in pre-planning, planning meetings and reviews.
  • Support in linking clients with assistance for Appeals and Reviews
